Desired Education Level: Doctorate degree in Physical Therapy from an accredited institution. Licensed as a Physical Therapist in the State of Idaho and Utah or the ability to obtain licensure.
Bear Lake Memorial Hospital is seeking a full time Physical Therapist to provide exceptional patient care and rehabilitation efforts and services.
RESPONSIBILITIES (This is not intended to be an exhaustive list of all responsibilities and duties required):
- The Physical Therapist delivers medically prescribed Physical Therapy in support of medical care as supported by research and evidence-based practice. Works with patients of all ages, race, creed, gender, and disability without bias or prejudice.
- Utilizes the evaluation of the patient’s rehabilitative needs to plan the care, implement and document effective, appropriate and safe Physical Therapy treatments.
- Helps patients and their families achieve the highest level of functioning.
- Works with other therapists, aides/techs regarding Physical Therapy Services.
- Participates in planning and helping to implement new services.
- Perform other related duties incidental to the work described herein.
- Adherence to confidentiality and privacy requirements.
- Adherence of safety policies and procedures.
- Must have the ability to contribute to the vision/culture of the most caring hospital.
Knowledge, Skills, Abilities and Other Requirements
Consistently works cooperatively with patients/residents, co-workers, physicians, consultants and ancillary service providers. Willing and able to be flexible and to work various shifts/hours/days. Excellent service and communication skills when interacting with co-workers, patients/residents and visitors. Must be kind and compassionate. Ability to read, understand, follow, and enforce safety policies and procedures. Good organizational skills. Must have a Doctorate degree in Physical Therapy from an accredited institution and preferably a minimum of 1-2 years’ experience in responsible therapy operations. Must have a current Idaho and Utah License as a Physical Therapist or the ability to obtain licensure.

Hospital Information
Bear Lake Memorial Hospital remains a stand-alone CAH hospital. In its 65+ year history, it has been extremely well supported by the citizens of the Bear Lake Valley both in terms of utilization and donations. BLMH was recognized as one of 32 hospitals (out of 1300) in the nation to outperform national financial benchmarks three consecutive years. In addition to offering the typical hospital services, e.g. in/outpatient, surgery, obstetrics, ER, swing beds, lab, diagnostic imaging, physical therapy, etc., Bear Lake Memorial Hospital also owns and operates: a 36 bed Skilled Nursing Facility, a 37 bed assisted living center, a 7 chair dialysis center, a mental health clinic, a home health agency, a fitness center and 4 physician clinics (Family Care, Internal Medicine, Orthopedic and Visiting Physician).
Therapy
The Therapy Department consists of three physical therapists, one physical therapy assistant, two speech and language pathologists, one occupational therapist, a therapy aide, and our clinic coordinator. Workload will consist of a mix of outpatient, inpatient, home health and skilled nursing facility services.
